Motor Vehicle Report Authorization

In connection with your application for employment or your continued employment, we may obtain a Motor Vehicle Report to verify your driving record. This Motor Vehicle Report may be obtained by Linkdrive Express, Inc, by our insurance agent (Dolliff, Inc.) or by the insurance company.

Please complete the form below and provide us with your authorization to procure this report. This authorization will be valid for one year from the date shown below.

I authorize my employer, insurance agent or insurance company to obtain a Motor Vehicle Report in connection with my application for employment or continued driving privileges and I release them from any liability as a result of such inquiry or disclosure.

I further waive my right to privacy in this regard and release and hold harmless my employer and its agents from any liability related to this waiver.
